Position Overview:
-Responds promptly to crisis situations, documents critical
incidents and is available, as needed, to clients
-Requires a minimum of 60 hours in home therapy per month
-Provides crisis/on call services and 24 hour, 7 days a week
support to assigned clients
-Facilitates an initial meeting with the client and the referral
source (as appropriate) within the required time frames
-Engages and works in partnership with the client to complete the
initial, comprehensive assessment, including a mental status exam,
resulting diagnosis based on the most recent Diagnostic and
Statistical Manual of Mental Disorders
-Develops treatment plan goals and objectives, in conjunction with
the client
-Provides individual, family and/or group counseling as appropriate
to client need
-Functions as a client advocate and liaison with other community
services
-Monitors client progress and service provision using the
established tools, and updates treatment plans based on those
assessments
-Maintains the client case record, documenting all client and
collateral contacts in a timely manner in the electronic medical
record
-Completes discharge summaries and aftercare contacts according to
agency policy
-Provides written and verbal progress reports to referring entities
within required time frames
-Works cooperatively with other program staff to ensure seamless,
effective services
-Participates in regular clinical supervision
-Travel to home-based therapy may require the employee to be able
to regularly drive to various DCCCA locations and other business
locations throughout the day including early morning or evening
hours
Requirements Include:
-Requires a Master's degree in human services or related field
-Requires 2 years of child welfare and/or family services
experience
-Requires licensure by the Behavioral Sciences Regulatory Board at
the master's level. Clinical licensure is preferred.
-Valid driver's license with a good driving record
-Must pass pre-employment background checks, reference checks,
fingerprinting and drug screening
